A "Sports Physical" is needed for ANY student (grades 6 - 8) participating in a school sponsored Intramural or Interscholastic sports activity.

If a student does not have a medical home (primary care physician), the district shall provide this examination at the school physician's office, free of charge. You will need the "school physician authorization" form to see the school doctor. This form is available at the school office.
